All library authorities running the Summer Reading Challenge are required to complete this evaluation form. *Please Note:* This is the standard library evaluation form. Library authorities taking part in the Cross-Authority Pilot Scheme will receive a different form. Please submit your evaluation form by email to srcevaluation@readingagency.org.uk by Thursday 14 October 2021. Send your evaluation form as an Excel spreadsheet and only submit one form per authority. We are unable to accept forms submitted as pdfs or Google sheets.
